Setting up bulk billing correctly from the start is critical to building a sustainable, patient-focused general practice. Bulk billing allows your patients to receive GP services at no out-of-pocket cost, funded entirely through Medicare rebates. For many practices, particularly those in lower socioeconomic or regional areas, bulk billing is not just a billing model but a core part of their value proposition to the community.

How Bulk Billing Works in General Practice

Bulk billing is the process where a GP accepts the Medicare rebate as full payment for a service, meaning the patient pays nothing. The GP assigns their right to the Medicare benefit to Services Australia, which then pays the rebate directly to the practice. This arrangement is governed by the Health Insurance Act 1973 and must comply with Medicare’s billing rules.

For the patient, bulk billing means no gap payment and no claim to submit. For the practice, it means guaranteed payment from Medicare (typically within days of lodgement) but at a fixed rate determined by the Medicare Benefits Schedule (MBS). Understanding the financial implications of bulk billing versus private billing models is essential before committing to a billing strategy.

Prerequisites for Bulk Billing

Medicare Provider Number

Every GP who bulk bills must hold a valid Medicare provider number for the specific practice location. The provider number links the practitioner to the practice address and is used for every Medicare claim submitted. Without a current provider number, you cannot submit bulk billing claims.

Practice Management Software

Efficient bulk billing requires practice management software that supports electronic claiming through Medicare’s Online Claiming system. The major practice management systems in Australia (Best Practice, Medical Director, Zedmed, Genie Solutions) all support electronic claiming. Your software must be correctly configured with each GP’s provider number, the practice location details, and the Medicare claiming channel.

Medicare Online Claiming Setup

To submit bulk billing claims electronically, your practice needs to be registered for Medicare Online Claiming (formerly ECLIPSE). This is set up through the HPOS portal and requires a site certificate and public key infrastructure (PKI) credentials. The setup process can take one to two weeks, so initiate it well before your practice opens for patients.

Bulk Billing Incentive Payments

The Australian Government provides Bulk Billing Incentive (BBI) payments to encourage GPs to bulk bill specific patient groups. These incentive payments are made in addition to the standard Medicare rebate and can significantly increase practice revenue.

The BBI applies to services provided to concession card holders (Commonwealth Seniors Health Card, Health Care Card, Pensioner Concession Card) and children under 16 years of age. The incentive amount varies depending on your practice location, with higher incentives available in regional, rural, and remote areas.

For practices in Modified Monash Model (MMM) 1 areas (major cities), the BBI is lower than for practices in MMM 2-7 areas. Understanding your MMM classification and the corresponding incentive rates is essential for accurate financial modelling. Practices that do not factor in BBI payments often underestimate their bulk billing revenue potential.

Financial Considerations for Bulk Billing Practices

Revenue Modelling

Before committing to a bulk billing model, build a detailed revenue model that accounts for the Medicare rebate for your most commonly billed items (MBS items 23, 36, 44 for standard consultations), BBI payments based on your patient demographics and location, the volume of patients you can realistically see per day, and overheads including rent, staff wages, consumables, and insurance.

A well-structured bulk billing practice can be highly profitable, but it requires higher patient volumes than a private billing practice to achieve the same revenue. The trade-off is a larger, more loyal patient base and simplified billing administration.

Maximising Revenue Within Bulk Billing

Practices can maximise their bulk billing revenue through several strategies. These include accurate MBS item selection (billing the correct item for each consultation length and complexity), claiming all eligible incentive payments, participating in the Practice Incentives Program (PIP), implementing chronic disease management plans (which attract additional Medicare items), and optimising appointment scheduling to reduce gaps and no-shows.

Compliance and Audit Considerations

Medicare compliance is non-negotiable for bulk billing practices. The Professional Services Review (PSR) scheme monitors Medicare claiming patterns and investigates practitioners whose billing is significantly above the peer average. Common compliance issues include billing longer consultation items than the service supports, inappropriate use of chronic disease management items, and failure to maintain adequate clinical records to justify claims.

Maintaining accurate, contemporaneous clinical records for every consultation is your primary defence against a Medicare audit. Each record should document the presenting complaint, clinical findings, diagnosis or assessment, and management plan, along with the consultation duration and complexity that supports the MBS item billed.
